Diagnosis: This is easy: simple biliary cysts. Comments: Biliary cysts are common (4% of liver sonograms) and are mainly encountered in middle-aged women . They may be congenital or, more rarely, acquired (post-traumatic, parasitic or inflammatory). They are usually solitary, but there can be several of them. The cyst is asymptomatic and is usually a chance finding; the mean size is 4 cm; however, there are also large cysts, up to 15 cm, that compress the bile ducts . Anatomically the cyst is a closed cavity which does not communicate with the biliary tree, is lined with biliary epithelium, and contains a clear liquid.

Diagnosis: This is facilitated by intravenous cholangiography, which shows a stone lodged in Vater's ampulla. The hyperechogenic structure corresponds to a huge angioma. Comments: The linear structure which seems intracholedocal is a pitfall. It is in fact the right hepatic artery which crosses the bile duct. If this is particularly protuberant it produces an indentation in the form of a hyperechogenic line, more visible when the bile duct is dilated. It should not be confused with lithiasis, the more so in that absence of acoustic shadow is not an absolute criterion (Case 28).

The bile duct is not visible, but at the level of the hilar junction there is a poorly delimited hyperechogenic formation , at which the dilated hepatic ducts terminate. The gallbladder is small and has thickened walls. Since the patient has fasted, this is a sig;:! of gallbladder dysfunction. Note the presence of gallstones (Scan C). The portal vein shows normal shape and calibre (Scan B); the pancreas is also normal (Scan D). Diagnosis: This is carcinoma of the main bile duct. Comments: Carcinomas of the bile duct in man classically occur between the ages of 60 and 80 years.
